Narrative:

A B727 landed on runway 9L at mia and was observed turning left and exiting the runway at intersection L7. A DC8 was then cleared for takeoff on runway 9L. The amass warning system alerted that the runway was occupied. The DC8 was told to cancel takeoff clearance 3 times, but the exact airline call sign was not read and the xmissions were weak due to a faulty microphone. The DC8 said, 'say again,' but departed anyway. The B727 had exited then went back on the runway and re-exited at intersection L8 just as the DC8 flew past intersection L8. Txwys L7 and L8 had barrels on them, confusing the B727 pilot. The B727 should have stopped at L7 and asked questions before going back on the runway. Luckily, no harm was done.

Title: NEAR GND COLLISION DURING A NIGHT OP BTWN A TAXIING B727 AND A DEP DC8 AT MIA, FL.

Narrative: A B727 LANDED ON RWY 9L AT MIA AND WAS OBSERVED TURNING L AND EXITING THE RWY AT INTXN L7. A DC8 WAS THEN CLRED FOR TKOF ON RWY 9L. THE AMASS WARNING SYS ALERTED THAT THE RWY WAS OCCUPIED. THE DC8 WAS TOLD TO CANCEL TKOF CLRNC 3 TIMES, BUT THE EXACT AIRLINE CALL SIGN WAS NOT READ AND THE XMISSIONS WERE WEAK DUE TO A FAULTY MIKE. THE DC8 SAID, 'SAY AGAIN,' BUT DEPARTED ANYWAY. THE B727 HAD EXITED THEN WENT BACK ON THE RWY AND RE-EXITED AT INTXN L8 JUST AS THE DC8 FLEW PAST INTXN L8. TXWYS L7 AND L8 HAD BARRELS ON THEM, CONFUSING THE B727 PLT. THE B727 SHOULD HAVE STOPPED AT L7 AND ASKED QUESTIONS BEFORE GOING BACK ON THE RWY. LUCKILY, NO HARM WAS DONE.
