Narrative:

Aircraft checked on frequency climbing to FL230. I reclred him to FL310. Later I reclred him to FL360. I thought I had given him FL350. I gave a position relief briefing and went on break. Shortly thereafter; climbing aircraft conflicted with an aircraft level at FL360. Human performance considerations: I was feeling less than 100%. I looked at requested altitude in data block and issued it; rather than altitude for separation.

Narrative: ACFT CHKED ON FREQ CLBING TO FL230. I RECLRED HIM TO FL310. LATER I RECLRED HIM TO FL360. I THOUGHT I HAD GIVEN HIM FL350. I GAVE A POS RELIEF BRIEFING AND WENT ON BREAK.
