Narrative:
On mar/wed/05; aircraft X was in ZZZ1 for layover check. The #4 main brake was found worn to limits. The #4 main brake was changed per the aircraft maintenance manual. On mar/thu/06; I was informed that aircraft X landed in ZZZ2 on flight abc and that the brake rod was found disconnected from #4 main brake and that the brake had spun resulting in damage to the hydraulic and electrical lines. When I was told this I thought what would make the brake spin on the axle. The first thing I thought of was that the pin had come loose of the brake rod. I know for a fact that the pin was installed because I installed the cotter pin for the bolt and castle nut that holds the pin in. The second thing that I thought of was that the end cap may not have been installed and by not installing this part although the bolt and nut were installed; the pin could just come out; but I cannot verify this and third the bolt just broke.

Title: A B757-200 WAS RELEASED FOR SVC AFTER #4 MAIN BRAKE WAS REPLACED. ON FIRST LNDG #4 BRAKE SPUN ON AXLE DUE TO BRAKE ROD NOT SECURED.
Narrative: ON MAR/WED/05; ACFT X WAS IN ZZZ1 FOR LAYOVER CHK. THE #4 MAIN BRAKE WAS FOUND WORN TO LIMITS. THE #4 MAIN BRAKE WAS CHANGED PER THE ACFT MAINT MANUAL. ON MAR/THU/06; I WAS INFORMED THAT ACFT X LANDED IN ZZZ2 ON FLT ABC AND THAT THE BRAKE ROD WAS FOUND DISCONNECTED FROM #4 MAIN BRAKE AND THAT THE BRAKE HAD SPUN RESULTING IN DAMAGE TO THE HYD AND ELECTRICAL LINES. WHEN I WAS TOLD THIS I THOUGHT WHAT WOULD MAKE THE BRAKE SPIN ON THE AXLE. THE FIRST THING I THOUGHT OF WAS THAT THE PIN HAD COME LOOSE OF THE BRAKE ROD. I KNOW FOR A FACT THAT THE PIN WAS INSTALLED BECAUSE I INSTALLED THE COTTER PIN FOR THE BOLT AND CASTLE NUT THAT HOLDS THE PIN IN. THE SECOND THING THAT I THOUGHT OF WAS THAT THE END CAP MAY NOT HAVE BEEN INSTALLED AND BY NOT INSTALLING THIS PART ALTHOUGH THE BOLT AND NUT WERE INSTALLED; THE PIN COULD JUST COME OUT; BUT I CANNOT VERIFY THIS AND THIRD THE BOLT JUST BROKE.
