Narrative:

At 3.5 mi out on final; we had a medical emergency. Relief pilot spoke to cabin and company. I declared medical emergency with tower. All went well except at gate. Gate was not ready and they were not hurrying. I left all exterior lights on and even though we had a red light (stay out of gate area) I crossed road and I taxied into first 1/3 of gate area. I'd hoped that my actions would be recognized as so unusual that an emergency exists. Ramp didn't increase their pace one bit. Paramedics waited in jetway until ramp personnel eventually readied gate and we received a 'green light' for taxi in.
