Narrative:
I taxied out to runway 18L at clt. While my first officer was making the 'flight attendants please take your seats announcement' the tower cleared me into position and hold. The tower told an air carrier flight beside me at spot X to taxi up and hold short and that they would follow the air carrier right out. I erroneously believed that I had been cleared for takeoff. When my first officer came back on communication #1; I told him we were cleared for takeoff. I made the takeoff. During the takeoff roll I had a moment of doubt; I asked on tower frequency 'is air carrier cleared for takeoff?' I received no response and continued the takeoff. When I arrived at destination ground control told me to call the tower in clt. I called the tower. The person I spoke with said that I had not been cleared for takeoff; that they had crossing traffic downfield that cleared safely. But this set off an alarm in the tower and they had to file a pilot deviation report. Supplemental information from acn 715443: it was the last day of a 4-DAY trip and the last round-trip departure. We had both been on duty for the day approximately 9 hours with 9 hours 40 mins of rest the night prior. Being the last trip out and back; it was possible that the PF was in a rush to complete the trip and go home. Inadequate rest or fatigue was probably not a contributing factor.
Original NASA ASRS Text
Title: A CL65 FLT CREW TOOK OFF WITHOUT CLRNC FROM CLT.
Narrative: I TAXIED OUT TO RWY 18L AT CLT. WHILE MY FO WAS MAKING THE 'FLT ATTENDANTS PLEASE TAKE YOUR SEATS ANNOUNCEMENT' THE TWR CLRED ME INTO POS AND HOLD. THE TWR TOLD AN ACR FLT BESIDE ME AT SPOT X TO TAXI UP AND HOLD SHORT AND THAT THEY WOULD FOLLOW THE ACR RIGHT OUT. I ERRONEOUSLY BELIEVED THAT I HAD BEEN CLRED FOR TKOF. WHEN MY FO CAME BACK ON COM #1; I TOLD HIM WE WERE CLRED FOR TKOF. I MADE THE TKOF. DURING THE TKOF ROLL I HAD A MOMENT OF DOUBT; I ASKED ON TWR FREQ 'IS ACR CLRED FOR TKOF?' I RECEIVED NO RESPONSE AND CONTINUED THE TKOF. WHEN I ARRIVED AT DEST GND CTL TOLD ME TO CALL THE TWR IN CLT. I CALLED THE TWR. THE PERSON I SPOKE WITH SAID THAT I HAD NOT BEEN CLRED FOR TKOF; THAT THEY HAD XING TFC DOWNFIELD THAT CLRED SAFELY. BUT THIS SET OFF AN ALARM IN THE TWR AND THEY HAD TO FILE A PLTDEV RPT. SUPPLEMENTAL INFO FROM ACN 715443: IT WAS THE LAST DAY OF A 4-DAY TRIP AND THE LAST ROUND-TRIP DEP. WE HAD BOTH BEEN ON DUTY FOR THE DAY APPROX 9 HRS WITH 9 HRS 40 MINS OF REST THE NIGHT PRIOR. BEING THE LAST TRIP OUT AND BACK; IT WAS POSSIBLE THAT THE PF WAS IN A RUSH TO COMPLETE THE TRIP AND GO HOME. INADEQUATE REST OR FATIGUE WAS PROBABLY NOT A CONTRIBUTING FACTOR.
