Narrative:

On day 5 of a series of 2 trips; the first trip had a reschedule that arrived at XA02 with a short overnight with short breaks with little chance to catch up on my rest until the last night with a 19 hour layover in ZZZ. The hotel was loud and my room; which I was unable to get moved; was next to the elevator. I was unable to get any undisturbed sleep. On approach to ZZZ1; I found myself falling asleep while flying an approach to minimums. I went to the flight office in ZZZ1; explained what happened and removed myself from the trip because I felt I was too tired to fly today without getting some rest.

Title: A B737 FATIGUED CAPT REMOVED HIMSELF FROM A TRIP AFTER FALLING ASLEEP ON APCH FOLLOWING NIGHTS OF MINIMUM REST AND POOR HOTEL ACCOMMODATIONS.

Narrative: ON DAY 5 OF A SERIES OF 2 TRIPS; THE FIRST TRIP HAD A RESCHEDULE THAT ARRIVED AT XA02 WITH A SHORT OVERNIGHT WITH SHORT BREAKS WITH LITTLE CHANCE TO CATCH UP ON MY REST UNTIL THE LAST NIGHT WITH A 19 HR LAYOVER IN ZZZ. THE HOTEL WAS LOUD AND MY ROOM; WHICH I WAS UNABLE TO GET MOVED; WAS NEXT TO THE ELEVATOR. I WAS UNABLE TO GET ANY UNDISTURBED SLEEP. ON APCH TO ZZZ1; I FOUND MYSELF FALLING ASLEEP WHILE FLYING AN APCH TO MINIMUMS. I WENT TO THE FLT OFFICE IN ZZZ1; EXPLAINED WHAT HAPPENED AND REMOVED MYSELF FROM THE TRIP BECAUSE I FELT I WAS TOO TIRED TO FLY TODAY WITHOUT GETTING SOME REST.
