#3 engine failed. Suspect ice frozen ball from forward lavatory drainage area was ingested when it broke loose from leaking drain. Problem handled per emergency checklist. To prevent recurrence: better preflting of forward lavatory drain (external) or re-engineer drain area.

Narrative: #3 ENG FAILED. SUSPECT ICE FROZEN BALL FROM FORWARD LAVATORY DRAINAGE AREA WAS INGESTED WHEN IT BROKE LOOSE FROM LEAKING DRAIN. PROB HANDLED PER EMER CHKLIST. TO PREVENT RECURRENCE: BETTER PREFLTING OF FORWARD LAVATORY DRAIN (EXTERNAL) OR RE-ENGINEER DRAIN AREA.
