Narrative:

Aircraft X was issued a turn direct cshel as per the CSHEL4 departure SID and given a climb to 080 due to crossing traffic at 090; normally the CSHEL4 departure will be given a climb to 100 then handed of to the lal hi sector of ZMA. Aircraft X was incorrectly given a frequency change to lal hi. Immediately after the mistake was made coordination was attempted with the receiving sector who gave their initials and got off of the land line before effective coordination could be completed. The lal hi controller then climbed aircraft X through the altitude of the converging traffic. The affected traffic was then given an altitude and heading to avoid a collision. Recommendation; ensure proper land line communications are preformed.

Title: RSW and ZMA Controllers described a loss of separation event when coordination between the two facilities regarding an altitude assignment was confused leading to the error.

Aircraft X was incorrectly given a frequency change to LAL HI. The LAL HI Controller then climbed Aircraft X through the altitude of the converging traffic. The affected traffic was then given an altitude and heading to avoid a collision.
