Narrative:

The local controller cleared medium large transport X onto runway 22R position and hold, traffic landing runway 31, us. Shortly thereafter, medium large transport Y that was holding short of runway 31 called ready. The controller responded in a confused manner mixing medium large transport Y and medium large transport X flts up. He began to clear medium large transport Y onto the runway in front of us. We were approximately 2 mi final. The controller realized his mistake after medium large transport X pilot reported holding in position on runway 22R. Medium large transport Y was then instructed to hold short of runway 31. The controller either lost his traffic picture, was distracted, or somehow became confused with only 3-4 airplanes under his control. This is becoming a daily occurrence.

Title: CTLR CLEARED AN MLG INTO POSITION ON A RWY ON WHICH AN LTT WAS LNDG. BEFORE ACFT COULD TAKE RWY, CTLR STOPPED THE MLG BEFORE SEPARATION WAS LOST.

Narrative: THE LCL CTLR CLRED MLG X ONTO RWY 22R POS AND HOLD, TFC LNDG RWY 31, US. SHORTLY THEREAFTER, MLG Y THAT WAS HOLDING SHORT OF RWY 31 CALLED READY. THE CTLR RESPONDED IN A CONFUSED MANNER MIXING MLG Y AND MLG X FLTS UP. HE BEGAN TO CLR MLG Y ONTO THE RWY IN FRONT OF US. WE WERE APPROX 2 MI FINAL. THE CTLR REALIZED HIS MISTAKE AFTER MLG X PLT RPTED HOLDING IN POS ON RWY 22R. MLG Y WAS THEN INSTRUCTED TO HOLD SHORT OF RWY 31. THE CTLR EITHER LOST HIS TFC PICTURE, WAS DISTRACTED, OR SOMEHOW BECAME CONFUSED WITH ONLY 3-4 AIRPLANES UNDER HIS CTL. THIS IS BECOMING A DAILY OCCURRENCE.
