Narrative:
Takeoff was made with AC packs off as per MEL. Right pack was turned on per MEL passing 500 ft AGL. During initial climbout; single pack operation would not adequately pressurize aircraft. 25 psi noted on right pack; cabin alt control lever automatic; and outflow valve full closed; cabin climbing. Leveled off at 5;000 ft. QRH abnormal checklist followed for pressurization flow light. Emergency declared due to overweight landing requirements. Captain performed overweight landing. Uneventful approach; landing; and return to gate.

Title: Dispatched with the left pack inoperative; an MD-80 was unable to pressurize after takeoff while operating on a single pack per the MEL and AOM requirements.
