Narrative:
During our initial brief in the blocks and subsequent clearance for departure we were distracted by outside inputs and missed key MCP inputs (altitude window; and tmc). During our taxi out we were distracted by many ATC instructions; which interrupted our takeoff brief. We again missed the selection of D-2 power setting and we missed MCP altitude window conformation. On takeoff roll we discovered the power (to; not to-2.) setting problem; continue. That distraction caused a late contact with departure radar. That combined with the wrong altitude in MCP (10;000 ft). We flew through the 4;000 ft level off restriction for the STAR. We were at 5;000 ft when the controller said; 'climb to FL240 and you should have leveled at 4;000 ft.'

Title: A B767 crew was very distracted during preflight and taxi which resulted in an incorrect takeoff power setting; failure to contact ATC after departure; and failing to level at 4;000 FT after takeoff.
Narrative: During our initial brief in the blocks and subsequent clearance for departure we were distracted by outside inputs and missed key MCP inputs (ALT window; and TMC). During our taxi out we were distracted by many ATC instructions; which interrupted our takeoff brief. We again missed the selection of D-2 power setting and we missed MCP ALT window conformation. On takeoff roll we discovered the power (TO; not TO-2.) setting problem; continue. That distraction caused a late contact with Departure radar. That combined with the wrong ALT in MCP (10;000 FT). We flew through the 4;000 FT level off restriction for the STAR. We were at 5;000 FT when the Controller said; 'Climb to FL240 and you should have leveled at 4;000 FT.'
