Narrative:
Both pilots had short sleep cycles. Early get up after short call on reserve. First officer's first flight after vacation. Low time in aircraft. OM inbound first officer flying. Received frequency change to 118.3 (tower) double frequency radio head changed frequency. Did not FLIP switch. Flaps 28 degree landing bug speed vref and 5 (132 KIAS). About this time first officer misinterpreted 28 degree maneuvering speed for 28 degree approach speed and dials in 142 KIAS for speed command. Captain distracted, reset speed to 132. Stabilized first officer, suggested idle power in the flare, monitored rollout, (a bit long landing). Turned off at end and realized still on approach and no landing clearance. Reported to tower. Both pilots fatigued. Relatively new captain, very new first officer. Captain distracted and concerned with first officer approach and landing. No real excuse. Things happen. 3 man crew could have helped. Callback conversation with reporter revealed the following: participated in the boeing study. Felt his main responsibility was to insure the first officer got the aircraft back on approach speed. No excuse for not confiming landing clearance.
