Narrative:

In holding pattern at 5000 ft, conflict aircraft also in holding at 4000 ft, requested clearance to divert to ewr. Conflict aircraft was pointed out by approach. We responded that we had the aircraft visually. Clearance was given to descend to 4000 ft maintain visual separation fly heading 220 degrees. Conflict aircraft was at our 1 O'clock position. We turned to heading 270 degrees in our descent to maintain visual separation. A clearance from approach to turn as necessary to maintain visual separation would have made the situation clrer in my mind. Supplemental information from acn 198792: TCASII gave TA and short RA asking for descent. Heavy cockpit load at time with desire to expedite divert added to confusion.
