Narrative:
Preflight and taxi normal. Awaiting takeoff clearance, all checklists complete. Tower issued instructions to aircraft to 'line up and wait.' we taxied aircraft onto runway only to be advised of an aircraft behind us on final. No room to exit runway. Aircraft behind us made a go around. What was apparently missed were the first part of instructions, which were 'after the aircraft on final, line up and wait.' we did not hear these instructions. I asked first officer prior to taxi onto runway, 'line up and wait?' that's what was relayed to tower. After this, the tower told us to remain on runway and proceed with a normal takeoff. Rest of flight uneventful. Fix: change taxi on and hold instructions to be standard around the world and eliminate 'after aircraft on final' instructions.

Title: RWY INCURSION.
Narrative: PREFLT AND TAXI NORMAL. AWAITING TKOF CLRNC, ALL CHKLISTS COMPLETE. TWR ISSUED INSTRUCTIONS TO ACFT TO 'LINE UP AND WAIT.' WE TAXIED ACFT ONTO RWY ONLY TO BE ADVISED OF AN ACFT BEHIND US ON FINAL. NO ROOM TO EXIT RWY. ACFT BEHIND US MADE A GAR. WHAT WAS APPARENTLY MISSED WERE THE FIRST PART OF INSTRUCTIONS, WHICH WERE 'AFTER THE ACFT ON FINAL, LINE UP AND WAIT.' WE DID NOT HEAR THESE INSTRUCTIONS. I ASKED FO PRIOR TO TAXI ONTO RWY, 'LINE UP AND WAIT?' THAT'S WHAT WAS RELAYED TO TWR. AFTER THIS, THE TWR TOLD US TO REMAIN ON RWY AND PROCEED WITH A NORMAL TKOF. REST OF FLT UNEVENTFUL. FIX: CHANGE TAXI ON AND HOLD INSTRUCTIONS TO BE STANDARD AROUND THE WORLD AND ELIMINATE 'AFTER ACFT ON FINAL' INSTRUCTIONS.
