Narrative:
Mar/xa/95 approximately XA30 received phone call, as assigned reserved, to report to newark airport to fly trip to san juan. This is not my normal trip or normal trip for 757 aircraft. Trip is normally scheduled for 727 aircraft. Copilot called in from north new york. Aircraft originally scheduled for domestic flight and moved at last minute to cover san juan. In rush to move aircraft, preflight, filing, loading, aircraft checks, etc, I verified maintenance checks for airworthiness and other pilot checks, along with ETOPS flight plan information. I overlooked that maintenance had not done the required sign off of maintenance. ETOPS checks flight operated normally, all system operated normally and checked accordingly. In postflt review 4 days later it was discovered that maintenance had not signed off special (automatic) ETOPS check and I failed to note sign off missing. Supplemental information from acn 299728: aircraft released by maintenance for flight and/but aircraft dispatched without ETOPS inspection. I thought it was signed off along with the normal airworthiness signature. Apparently maintenance overlooked the requirement because of a last min aircraft swap. Flight was problem/occurrence free and for only 3 mins was in a 75 min ETOPS realm. The balance was 60 mins or less.

Title: ETOPS SIGNOFF FOR BERMUDA FLT NOT ACCOMPLISHED.
Narrative: MAR/XA/95 APPROX XA30 RECEIVED PHONE CALL, AS ASSIGNED RESERVED, TO RPT TO NEWARK ARPT TO FLY TRIP TO SAN JUAN. THIS IS NOT MY NORMAL TRIP OR NORMAL TRIP FOR 757 ACFT. TRIP IS NORMALLY SCHEDULED FOR 727 ACFT. COPLT CALLED IN FROM N NEW YORK. ACFT ORIGINALLY SCHEDULED FOR DOMESTIC FLT AND MOVED AT LAST MINUTE TO COVER SAN JUAN. IN RUSH TO MOVE ACFT, PREFLT, FILING, LOADING, ACFT CHKS, ETC, I VERIFIED MAINT CHKS FOR AIRWORTHINESS AND OTHER PLT CHKS, ALONG WITH ETOPS FLT PLAN INFO. I OVERLOOKED THAT MAINT HAD NOT DONE THE REQUIRED SIGN OFF OF MAINT. ETOPS CHKS FLT OPERATED NORMALLY, ALL SYS OPERATED NORMALLY AND CHKED ACCORDINGLY. IN POSTFLT REVIEW 4 DAYS LATER IT WAS DISCOVERED THAT MAINT HAD NOT SIGNED OFF SPECIAL (AUTOMATIC) ETOPS CHK AND I FAILED TO NOTE SIGN OFF MISSING. SUPPLEMENTAL INFO FROM ACN 299728: ACFT RELEASED BY MAINT FOR FLT AND/BUT ACFT DISPATCHED WITHOUT ETOPS INSPECTION. I THOUGHT IT WAS SIGNED OFF ALONG WITH THE NORMAL AIRWORTHINESS SIGNATURE. APPARENTLY MAINT OVERLOOKED THE REQUIREMENT BECAUSE OF A LAST MIN ACFT SWAP. FLT WAS PROB/OCCURRENCE FREE AND FOR ONLY 3 MINS WAS IN A 75 MIN ETOPS REALM. THE BAL WAS 60 MINS OR LESS.
