Narrative:
An unk small jet fighter type trainer (red) joined in formation with us for approximately 5 mins, then departed the area. Callback conversation with reporter revealed the following information: reporter stated that the other aircraft was a privately owned military gnat jet trainer. He stated that the fwa ATC approach personnel advised that they had been told that the formation flight had been prearranged and that the other aircraft was not on a flight plan. The captain suspected that the arrangement, if made, was between his first officer and the other aircraft pilot. He further stated that the first officer had been off flying status for a period of time, and thinks that it may have been connected to this incident. He said that he received a letter from the FAA asking for more information as a result of the near midair collision report that he had submitted.

Title: NMAC BTWN AN LGT DSNDING AND AN MTR JOINING UP ALONGSIDE IN A FORMATION FLT FOR A FEW MINS.
Narrative: AN UNK SMALL JET FIGHTER TYPE TRAINER (RED) JOINED IN FORMATION WITH US FOR APPROX 5 MINS, THEN DEPARTED THE AREA. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: RPTR STATED THAT THE OTHER ACFT WAS A PRIVATELY OWNED MIL GNAT JET TRAINER. HE STATED THAT THE FWA ATC APCH PERSONNEL ADVISED THAT THEY HAD BEEN TOLD THAT THE FORMATION FLT HAD BEEN PREARRANGED AND THAT THE OTHER ACFT WAS NOT ON A FLT PLAN. THE CAPT SUSPECTED THAT THE ARRANGEMENT, IF MADE, WAS BTWN HIS FO AND THE OTHER ACFT PLT. HE FURTHER STATED THAT THE FO HAD BEEN OFF FLYING STATUS FOR A PERIOD OF TIME, AND THINKS THAT IT MAY HAVE BEEN CONNECTED TO THIS INCIDENT. HE SAID THAT HE RECEIVED A LETTER FROM THE FAA ASKING FOR MORE INFO AS A RESULT OF THE NMAC RPT THAT HE HAD SUBMITTED.
