Narrative:

In aircraft log flight attendant jump seat xx was placarded inoperative due to seat belt assembly. Takeoff performance system showed full load of 22F 166Y. No jump seat riders in cockpit or cabin. #1 flight attendant reported during cabin ready report 5 flight attendants, no mention of jump seat riders. Agent gave seats to 2 flight attendants deadheading. The boarding pass for xx should not have been issued. After checking the closeout, all takeoff weights checked as planned load 188 passenger. The form for flight attendant's jump seat was overlooked. Later in the flight we noticed the form on the closeout. After further investigation the flight attendant in xx said seat belt was shorter than normal but still fit her. She was secure in seat for takeoff and landing. Why did the agent's computer block out xx? Why was a boarding pass issued? Xx was placarded since oct/xx/97 -- 1 full day before our flight.

Title: B757 ACFT PRIOR TO FLT WITH FULL PAX LOAD. PAX AGENT BOARDED FLT ATTENDANT TO SIT IN JUMP SEAT WHICH WAS DEFERRED AS INOP BECAUSE OF THE SEAT BELT ASSEMBLY. FLT ATTENDANT SAID THE BELT FIT.

NO JUMP SEAT RIDERS IN COCKPIT OR CABIN. #1 FLT ATTENDANT RPTED DURING CABIN READY RPT 5 FLT ATTENDANTS, NO MENTION OF JUMP SEAT RIDERS. AGENT GAVE SEATS TO 2 FLT ATTENDANTS DEADHEADING. THE BOARDING PASS FOR XX SHOULD NOT HAVE BEEN ISSUED. AFTER CHKING THE CLOSEOUT, ALL TKOF WTS CHKED AS PLANNED LOAD 188 PAX. THE FORM FOR FLT ATTENDANT'S JUMP SEAT WAS OVERLOOKED. LATER IN THE FLT WE NOTICED THE FORM ON THE CLOSEOUT. AFTER FURTHER INVESTIGATION THE FLT ATTENDANT IN XX SAID SEAT BELT WAS SHORTER THAN NORMAL BUT STILL FIT HER. SHE WAS SECURE IN SEAT FOR TKOF AND LNDG. WHY DID THE AGENT'S COMPUTER BLOCK OUT XX? WHY WAS A BOARDING PASS ISSUED? XX WAS PLACARDED SINCE OCT/XX/97 -- 1 FULL DAY BEFORE OUR FLT.
