Narrative:

Departed pvd en route to jfk-mem. Hydraulic service panel door was lost en route. So noted panel was open during walkaround at jfk. Aircraft was being fueled and he decided the mechanics were also servicing the hydraulics. So he continued his preflight. Panel was reported to me as being missing when we arrived at mem. So was on his second line trip and really didn't have a good feel for what was routine and failed to mention the open door to anyone. Mechanics should have checked the aircraft after pushback. They reported all doors closed. The door may, in fact, have been closed at jfk and departed the aircraft by becoming unlatched in light turbulence. Supplemental information from acn 412536: the flight was scheduled from pvd to jfk to mem. My walkaround in pvd was uneventful. In jfk, I noticed that the hydraulic service door was open, and I assumed that maintenance was servicing the hydraulics or checking something out. I did not speak to maintenance or the captain about the open door. The door was found missing when we arrived in memphis. I believe that it bounced open taxiing or taking off in new york (jfk runway 13L) and then blew off en route to memphis.

Title: THE SO ON A B727 FAILS TO CLOSE THE HYD SVC ACCESS DOOR WHEN IT IS NOTED TO BE OPEN DURING HIS GND PREFLT AT PVD. DOOR ENDS UP MISSING AT MEM.

Narrative: DEPARTED PVD ENRTE TO JFK-MEM. HYD SVC PANEL DOOR WAS LOST ENRTE. SO NOTED PANEL WAS OPEN DURING WALKAROUND AT JFK. ACFT WAS BEING FUELED AND HE DECIDED THE MECHS WERE ALSO SVCING THE HYDS. SO HE CONTINUED HIS PREFLT. PANEL WAS RPTED TO ME AS BEING MISSING WHEN WE ARRIVED AT MEM. SO WAS ON HIS SECOND LINE TRIP AND REALLY DIDN'T HAVE A GOOD FEEL FOR WHAT WAS ROUTINE AND FAILED TO MENTION THE OPEN DOOR TO ANYONE. MECHS SHOULD HAVE CHKED THE ACFT AFTER PUSHBACK. THEY RPTED ALL DOORS CLOSED. THE DOOR MAY, IN FACT, HAVE BEEN CLOSED AT JFK AND DEPARTED THE ACFT BY BECOMING UNLATCHED IN LIGHT TURB. SUPPLEMENTAL INFO FROM ACN 412536: THE FLT WAS SCHEDULED FROM PVD TO JFK TO MEM. MY WALKAROUND IN PVD WAS UNEVENTFUL. IN JFK, I NOTICED THAT THE HYD SVC DOOR WAS OPEN, AND I ASSUMED THAT MAINT WAS SVCING THE HYDS OR CHKING SOMETHING OUT. I DID NOT SPEAK TO MAINT OR THE CAPT ABOUT THE OPEN DOOR. THE DOOR WAS FOUND MISSING WHEN WE ARRIVED IN MEMPHIS. I BELIEVE THAT IT BOUNCED OPEN TAXIING OR TAKING OFF IN NEW YORK (JFK RWY 13L) AND THEN BLEW OFF ENRTE TO MEMPHIS.
