Narrative:
Crew discovered anti-ice generator problem en route to maf, and the crew placarded the generator with aid of MEL, dispatch, and maintenance. Both dispatch and maintenance were notified. We were assigned a different aircraft upon our return to ZZZ, and maintenance took the aircraft logbook. We were reassigned to the previous aircraft (xyz), just before departure. Since maintenance still had the maintenance logbook, we assumed that they would sign the write-up off in the logbook. Logbook was returned to crew with page with write-up folded. We thought he was through with it. Unfortunately, he had not signed off the write-up, and, not noticing, we took off for cll. After a normal flight to cll, aircraft was parked. Open write-up was discovered by crew in am.
Original NASA ASRS Text
Title: A SAAB 340D WAS DISPATCHED IN NON COMPLIANCE WITH AN OPEN UNANSWERED LOGBOOK WRITE-UP CAUSED BY NO LOGBOOK INSPECTION BY MAINT.
Narrative: CREW DISCOVERED ANTI-ICE GENERATOR PROB ENRTE TO MAF, AND THE CREW PLACARDED THE GENERATOR WITH AID OF MEL, DISPATCH, AND MAINT. BOTH DISPATCH AND MAINT WERE NOTIFIED. WE WERE ASSIGNED A DIFFERENT ACFT UPON OUR RETURN TO ZZZ, AND MAINT TOOK THE ACFT LOGBOOK. WE WERE REASSIGNED TO THE PREVIOUS ACFT (XYZ), JUST BEFORE DEP. SINCE MAINT STILL HAD THE MAINT LOGBOOK, WE ASSUMED THAT THEY WOULD SIGN THE WRITE-UP OFF IN THE LOGBOOK. LOGBOOK WAS RETURNED TO CREW WITH PAGE WITH WRITE-UP FOLDED. WE THOUGHT HE WAS THROUGH WITH IT. UNFORTUNATELY, HE HAD NOT SIGNED OFF THE WRITE-UP, AND, NOT NOTICING, WE TOOK OFF FOR CLL. AFTER A NORMAL FLT TO CLL, ACFT WAS PARKED. OPEN WRITE-UP WAS DISCOVERED BY CREW IN AM.
