Narrative:
The captain turned on the seatbelt sign, as it was getting a little bumpy as we were nearing arizona. During compliance check, a passenger refused to put on her seatbelt. She stated she would buckle it when we land. I advised the captain had turned it on and that she was violating an far. She still would not comply. I inquired her name and she ignored me. I notified the #1 flight attendant, and during this call, for attendant #5 also advised her to put on her seatbelt. Flight attendant #1 came back and requested the passenger to comply, but she wouldn't and she notified the passenger. The passenger refused to give her name and ignored all the flight attendants who instructed her. Flight attendant #1 was unable to give a misconduct form without the name and then the captain made a PA to prepare for landing. The passenger then put her seatbelt on for landing. Another passenger seated near the event advised us this woman kept asking to move from the window seat and then wanted the aisle seat and then would change her mind again. She gave me her name and phone number if needed as a witness to the event. This information was passed along to flight attendant #1. Flight attendant #1 and myself checked the computer upon landing in attempt to get the passenger's name since she refused. This information was included in flight attendant #1's report. Other passenger seated in the rear of the aircraft witnessed the event and 1 male passenger came to us and stated this woman should be arrested, as she was not following our instructions and shouldn't be allowed to fly. I unfortunately did not get this man's name. I believe the witnessing of this incident was unsettling to those around it. Callback conversation with reporter revealed the following information: the reporter stated that the purser didn't ask for the captain to have auths meet the flight. The defiant woman appeared to be mentally unstable, according to the woman who kept switching seats with her, until the witness refused to move anymore. The woman had kept moving from window seat to aisle, back to window seat.

Title: CABIN ATTENDANT RPT, B757, ORD-PHX. WOMAN REFUSED TO PUT ON SEATBELT DURING TURB, REFUSED TO GIVE NAME. DID BUCKLE IT FOR LNDG.
Narrative: THE CAPT TURNED ON THE SEATBELT SIGN, AS IT WAS GETTING A LITTLE BUMPY AS WE WERE NEARING ARIZONA. DURING COMPLIANCE CHK, A PAX REFUSED TO PUT ON HER SEATBELT. SHE GAVE ME HER NAME AND PHONE NUMBER IF NEEDED AS A WITNESS TO THE EVENT. THIS INFO WAS PASSED ALONG TO FLT ATTENDANT #1. FLT ATTENDANT #1 AND MYSELF CHKED THE COMPUTER UPON LNDG IN ATTEMPT TO GET THE PAX'S NAME SINCE SHE REFUSED. THIS INFO WAS INCLUDED IN FLT ATTENDANT #1'S RPT. OTHER PAX SEATED IN THE REAR OF THE ACFT WITNESSED THE EVENT AND 1 MALE PAX CAME TO US AND STATED THIS WOMAN SHOULD BE ARRESTED, AS SHE WAS NOT FOLLOWING OUR INSTRUCTIONS AND SHOULDN'T BE ALLOWED TO FLY. I UNFORTUNATELY DID NOT GET THIS MAN'S NAME. I BELIEVE THE WITNESSING OF THIS INCIDENT WAS UNSETTLING TO THOSE AROUND IT. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: THE RPTR STATED THAT THE PURSER DIDN'T ASK FOR THE CAPT TO HAVE AUTHS MEET THE FLT. THE DEFIANT WOMAN APPEARED TO BE MENTALLY UNSTABLE, ACCORDING TO THE WOMAN WHO KEPT SWITCHING SEATS WITH HER, UNTIL THE WITNESS REFUSED TO MOVE ANYMORE. THE WOMAN HAD KEPT MOVING FROM WINDOW SEAT TO AISLE, BACK TO WINDOW SEAT.
