Narrative:
On oct/xa/00 I was working overtime on day shift. I got a gate call at terminal X gate Z on aircraft xyz, a B757. When a flight crew/cabin service cleaner told me about a cracked window at row 30F, I inspected/checked the inside of window. It was cracked, but could not feel because there is a plastic cover in front of the window pane. So, I inspected/checked the outside window pane. It was good, not cracked -- it was the inner pane that was cracked. I was on short time, so I called maintenance services from jetbridge and asked about cracks on cabin passenger windowas. Maintenance told me he could not find anything about it. Then maintenance said he was going to ask someone higher than him. Then maintenance told me to defer it to abc. I told him it was the inner pane. Well, I deferred it to general procedures manual abc. Well, that night, I checked the B757 maintenance manual. It called to replace it immediately. I called technician right away and told him the problem. Technician told me he was going to have XXX change it. Needless to say, I should have personally checked myself.

Title: A B757-200 WAS DISPATCHED IN NON COMPLIANCE WITH A DEFERRED ITEM ON A PAX WINDOW INNER PANE CRACKED IN CONFLICT WITH THE GENERAL PROCS MANUAL.
Narrative: ON OCT/XA/00 I WAS WORKING OVERTIME ON DAY SHIFT. I GOT A GATE CALL AT TERMINAL X GATE Z ON ACFT XYZ, A B757. WHEN A FLC/CABIN SVC CLEANER TOLD ME ABOUT A CRACKED WINDOW AT ROW 30F, I INSPECTED/CHKED THE INSIDE OF WINDOW. IT WAS CRACKED, BUT COULD NOT FEEL BECAUSE THERE IS A PLASTIC COVER IN FRONT OF THE WINDOW PANE. SO, I INSPECTED/CHKED THE OUTSIDE WINDOW PANE. IT WAS GOOD, NOT CRACKED -- IT WAS THE INNER PANE THAT WAS CRACKED. I WAS ON SHORT TIME, SO I CALLED MAINT SVCS FROM JETBRIDGE AND ASKED ABOUT CRACKS ON CABIN PAX WINDOWAS. MAINT TOLD ME HE COULD NOT FIND ANYTHING ABOUT IT. THEN MAINT SAID HE WAS GOING TO ASK SOMEONE HIGHER THAN HIM. THEN MAINT TOLD ME TO DEFER IT TO ABC. I TOLD HIM IT WAS THE INNER PANE. WELL, I DEFERRED IT TO GENERAL PROCS MANUAL ABC. WELL, THAT NIGHT, I CHKED THE B757 MAINT MANUAL. IT CALLED TO REPLACE IT IMMEDIATELY. I CALLED TECHNICIAN RIGHT AWAY AND TOLD HIM THE PROB. TECHNICIAN TOLD ME HE WAS GOING TO HAVE XXX CHANGE IT. NEEDLESS TO SAY, I SHOULD HAVE PERSONALLY CHKED MYSELF.
