Primary Problem | Passenger Human Performance |
Narrative:
Approximately 20 mins after takeoff, a smoke alarm went off in lavatory 3L. A passenger was observed smoking in the lavatory, then barricaded himself inside and would not come out, ignoring commands in japanese and english. The first officer took the door off the hinges, and the passenger still would not respond. The passenger appeared intoxicated and the crew confiscated 1/2 bottle of scotch from the passenger seat. The captain requested his cigarettes, and the passenger ignored his request. The captain landed the plane in nrt, and the passenger was ejected from the aircraft. Callback conversation with reporter revealed the following information: the reporter said that they had 2 relief pilots, and that a relief pilot was flying the plane, while the captain and first officer were out of the cockpit confronting the passenger. The captain and the reporter, who was the purser, were there when the first officer took the door off the hinges, and the captain demanded that the cigarettes be handed over. After that, he did finally come out of the lavatory without physical resistance. They returned to narita to have him taken off. An entourage of auths and police came on board, and it took 10 mins to coax the man off. He was an executive for a major newspaper, so the auths showed some deference to him, and that is probably why they didn't physically handcuff him and take him off.

Title: CABIN ATTENDANT RPT, B777, NRT-SEA. PAX SMOKING, BARRICADED HIMSELF IN LAVATORY, DRINKING FROM OWN BOTTLE. FO TOOK LAVATORY DOOR OFF HINGES. RETURN TO NRT, PAX REMOVED.
Narrative: APPROX 20 MINS AFTER TKOF, A SMOKE ALARM WENT OFF IN LAVATORY 3L. A PAX WAS OBSERVED SMOKING IN THE LAVATORY, THEN BARRICADED HIMSELF INSIDE AND WOULD NOT COME OUT, IGNORING COMMANDS IN JAPANESE AND ENGLISH. THE FO TOOK THE DOOR OFF THE HINGES, AND THE PAX STILL WOULD NOT RESPOND. THE PAX APPEARED INTOXICATED AND THE CREW CONFISCATED 1/2 BOTTLE OF SCOTCH FROM THE PAX SEAT. THE CAPT REQUESTED HIS CIGARETTES, AND THE PAX IGNORED HIS REQUEST. THE CAPT LANDED THE PLANE IN NRT, AND THE PAX WAS EJECTED FROM THE ACFT. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: THE RPTR SAID THAT THEY HAD 2 RELIEF PLTS, AND THAT A RELIEF PLT WAS FLYING THE PLANE, WHILE THE CAPT AND FO WERE OUT OF THE COCKPIT CONFRONTING THE PAX. THE CAPT AND THE RPTR, WHO WAS THE PURSER, WERE THERE WHEN THE FO TOOK THE DOOR OFF THE HINGES, AND THE CAPT DEMANDED THAT THE CIGARETTES BE HANDED OVER.
