Narrative:
Engine #2 failed to light off on start. After following procedures, a good start was attained using the other ignitor. Recalling a recent similar situation when I called our maintenance control and was told to 'wright it up on arrival,' I decided to do just that. I realized after we were airborne that I had missed a key part of that same MEL deferral procedure -- namely to notify maintenance control before departure. Had I taken the time to review the MEL book rather than relying on memory, this could have been avoided.

Title: DC10 FLT CREW DEPARTS ZZZZ, FO, WITH OPEN LOGBOOK ENTRY DUE TO IMPROPER DOCUMENTATION.
Narrative: ENG #2 FAILED TO LIGHT OFF ON START. AFTER FOLLOWING PROCS, A GOOD START WAS ATTAINED USING THE OTHER IGNITOR. RECALLING A RECENT SIMILAR SIT WHEN I CALLED OUR MAINT CTL AND WAS TOLD TO 'WRIGHT IT UP ON ARR,' I DECIDED TO DO JUST THAT. I REALIZED AFTER WE WERE AIRBORNE THAT I HAD MISSED A KEY PART OF THAT SAME MEL DEFERRAL PROC -- NAMELY TO NOTIFY MAINT CTL BEFORE DEP.
