Narrative:

Aircraft X and aircraft Y departed ord. Aircraft X was a non tracked target and was directly on top of aircraft Y. Due to miscom with C90 (ord approach), the wrong target was idented. Aircraft X, at 12000 ft, was climbed to 15000 ft. I, as the radar controller, was unaware of aircraft Y being directly on top of aircraft X. Shortly after the clearance, descended back to 12000 ft. Aircraft Y turned to a heading of 120 degrees for the traffic. I then reclred aircraft Y to 12000 ft and turned the aircraft nebound. Aircraft Y was idented and climbed to 14000 ft. Aircraft X was eventually climbed to FL230 and put back on course and handed off to the high sector.

Title: ZAU AND C90 INCORRECTLY XFER COM AND RADAR BTWN A DEPARTING E145 AND B737 WHICH CONFLICT AT 13000 FT.

Narrative: ACFT X AND ACFT Y DEPARTED ORD. ACFT X WAS A NON TRACKED TARGET AND WAS DIRECTLY ON TOP OF ACFT Y. DUE TO MISCOM WITH C90 (ORD APCH), THE WRONG TARGET WAS IDENTED. ACFT X, AT 12000 FT, WAS CLBED TO 15000 FT. I, AS THE RADAR CTLR, WAS UNAWARE OF ACFT Y BEING DIRECTLY ON TOP OF ACFT X. SHORTLY AFTER THE CLRNC, DSNDED BACK TO 12000 FT. ACFT Y TURNED TO A HDG OF 120 DEGS FOR THE TFC. I THEN RECLRED ACFT Y TO 12000 FT AND TURNED THE ACFT NEBOUND. ACFT Y WAS IDENTED AND CLBED TO 14000 FT. ACFT X WAS EVENTUALLY CLBED TO FL230 AND PUT BACK ON COURSE AND HANDED OFF TO THE HIGH SECTOR.
