Narrative:

I have been having perpetual problems with what I think is the accuracy of the commercial chart revisions. Often times there are duplicate pages or pages missing. This time I had complete duplication of the north america pages; and no asia pacific polar pages in the revision. This is a flight safety issue and must be addressed. Now that the revisions come into our mail boxes again it is safe to assume that they are not tampered with by other pilots taking out individual pages that they are missing. Once again; it seems to be an ongoing problem that not only has flight safety consequences; but possible FAA issues as well. Callback conversation with reporter revealed the following information: reporter advises the revisions for flight crew flight and operation manuals are provided by a commercial supplier. Revisions are placed in the individual pilot's mail box by administrative personnel directly from shipments of same from the supplier. He feels the variety of errors (ie not only missing pages; but multiple copies of the same page; complete sections missing or duplicated; etc) indicates the fault lies with the supplier and occurs during the compilation and collating of the revisions into their individual envelopes.

Title: A B747-400 PLT RPTS THAT AERO CHART REVISIONS FROM HIS ACR'S COMMERCIAL CHART SUPPLIER ARE OFTEN INCOMPLETE AND/OR HAVE EXTRA PAGES.

NOW THAT THE REVISIONS COME INTO OUR MAIL BOXES AGAIN IT IS SAFE TO ASSUME THAT THEY ARE NOT TAMPERED WITH BY OTHER PLTS TAKING OUT INDIVIDUAL PAGES THAT THEY ARE MISSING. ONCE AGAIN; IT SEEMS TO BE AN ONGOING PROB THAT NOT ONLY HAS FLT SAFETY CONSEQUENCES; BUT POSSIBLE FAA ISSUES AS WELL. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: RPTR ADVISES THE REVISIONS FOR FLT CREW FLT AND OP MANUALS ARE PROVIDED BY A COMMERCIAL SUPPLIER. REVISIONS ARE PLACED IN THE INDIVIDUAL PLT'S MAIL BOX BY ADMINISTRATIVE PERSONNEL DIRECTLY FROM SHIPMENTS OF SAME FROM THE SUPPLIER.
