Narrative:
Crew called out a worn tire on a crj-200 aircraft. During installation phase of tire replacement; forgot to install spacer on left outboard main tire. Plane went on to fly one leg and stayed overnight at a non-maintenance outstation. Detected fault in morning and immediately contacted supervisor and maintenance control. Maintenance control advised that the plane was scheduled on a flight; on first flight of the day. Advised line maintenance crew to install missing spacer on left outboard main tire when aircraft arrived. More concentration on task at hand to prevent memory lapses.

Title: A Mechanic reports he replaced the left outboard main tire on a CRJ-200; but forgot to re-install the axle nut washer. Aircraft had already departed; when he noticed the washer stuck to wheel bearing of the worn tire he had removed.
