Narrative:
[We had an] anti-ice failure after takeoff in icing conditions after two previous write-ups by same captain on same aircraft the day before. Pencil-whipped after two previous write-ups and signed off a critical safety system requiring an emergency air-return. Captain previously put on a maintenance 'list' for too many write-ups and this fact complicated his outlook regarding this aircraft's problems.
