Narrative:
Ord to ewr. Going into ewr, approach on 128.57 instructed us to cross sweet at 7000 (fqm 1 arrival). I, the PNF, read back the clearance and set 7000 in the altitude alert. About 3-4 mins later, approach gave us a new frequency of 133.5. When I checked in we were over the VOR at 11000 and approach asked if we received the crossing restriction at sweet. The captain stated he thought it was sweet at 11000, which was the altitude we were at. He said he did not verify this with the altitude alert. I, however, did not confirm with the captain that he knew the clearance, nor did I monitor the flight as necessary. I believe the main factor affecting this event is very simple. The crew was involved in conversation, not paying enough attention to the flight. We were above 10000 but nevertheless on approach control. More discipline was required!

Title: FLC OF AN LGT ACR ACFT FAILED TO MEET A XING ALT RESTRICTION.
Narrative: ORD TO EWR. GOING INTO EWR, APCH ON 128.57 INSTRUCTED US TO CROSS SWEET AT 7000 (FQM 1 ARR). I, THE PNF, READ BACK THE CLRNC AND SET 7000 IN THE ALT ALERT. I, HOWEVER, DID NOT CONFIRM WITH THE CAPT THAT HE KNEW THE CLRNC, NOR DID I MONITOR THE FLT AS NECESSARY. I BELIEVE THE MAIN FACTOR AFFECTING THIS EVENT IS VERY SIMPLE. THE CREW WAS INVOLVED IN CONVERSATION, NOT PAYING ENOUGH ATTN TO THE FLT. WE WERE ABOVE 10000 BUT NEVERTHELESS ON APCH CTL. MORE DISCIPLINE WAS REQUIRED!
