Narrative:
Thought deicing had been completed when jetway was returned to aircraft. Turned on pack and immediately got fumes. Turned off pack just as aft flight attendant called to say there was smoke in cabin. Told flight attendant we had fixed problem, don't evacuate/evacuation, keep me informed. Smoke dissipated. Turns out they were deicing the tail by the APU inlet. We caught the problem before there was enough smoke or fumes to need an evacuate/evacuation and before any passenger tried to self initiate one. (I had also made an immediate PA to the passenger explaining the problem.) we called maintenance control, MEL'ed the pack, and used the left pack only off the engine to avoid using any contaminated air from APU bleed duct. Maintenance ran right pack without problem at destination. The obvious problem was not assuring/verifying that deicing was done. I set myself up for it by not insisting on standard deicing phraseology at start of operation. Ground crew had not used the standard callouts at start. I saw the jetway, thought they were done for now and would call me later.
Original NASA ASRS Text
Title: FLC OF AN LGT PARKED AT THE GATE TURNED ON BLEED AIR FROM APU WHEN THE ACFT TAIL AREA WAS BEING DEICED RESULTING IN SMOKE AND FUMES COMING INTO THE CABIN. THE BLEED AIR WAS TURNED OFF AT ONCE AND THE SMOKE STOPPED.
Narrative: THOUGHT DEICING HAD BEEN COMPLETED WHEN JETWAY WAS RETURNED TO ACFT. TURNED ON PACK AND IMMEDIATELY GOT FUMES. TURNED OFF PACK JUST AS AFT FLT ATTENDANT CALLED TO SAY THERE WAS SMOKE IN CABIN. TOLD FLT ATTENDANT WE HAD FIXED PROB, DON'T EVAC, KEEP ME INFORMED. SMOKE DISSIPATED. TURNS OUT THEY WERE DEICING THE TAIL BY THE APU INLET. WE CAUGHT THE PROB BEFORE THERE WAS ENOUGH SMOKE OR FUMES TO NEED AN EVAC AND BEFORE ANY PAX TRIED TO SELF INITIATE ONE. (I HAD ALSO MADE AN IMMEDIATE PA TO THE PAX EXPLAINING THE PROB.) WE CALLED MAINT CTL, MEL'ED THE PACK, AND USED THE L PACK ONLY OFF THE ENG TO AVOID USING ANY CONTAMINATED AIR FROM APU BLEED DUCT. MAINT RAN R PACK WITHOUT PROB AT DEST. THE OBVIOUS PROB WAS NOT ASSURING/VERIFYING THAT DEICING WAS DONE. I SET MYSELF UP FOR IT BY NOT INSISTING ON STANDARD DEICING PHRASEOLOGY AT START OF OP. GND CREW HAD NOT USED THE STANDARD CALLOUTS AT START. I SAW THE JETWAY, THOUGHT THEY WERE DONE FOR NOW AND WOULD CALL ME LATER.
