Narrative:

During climb the so noticed the oil qty in the #2 engine decreasing. Decision was made to return to cts. Appropriate checklists for oil qty loss and engine shutdown were accomplished. 17000 pounds of fuel dumped. Engine was shut down when oil qty reached .75 gals. Engine windmilled for 6 mins. Emergency was declared. Uneventful landing. Maintenance found a threaded plug for a service port on the engine driven gear box to be stripped causing the bolt to be loose. The corrective action would be for maintenance to avoid 'xthreading' or stripping bolts when installed, or correctly replacing or repairing the stripped bolt or socket.

UNEVENTFUL LNDG. MAINT FOUND A THREADED PLUG FOR A SVC PORT ON THE ENG DRIVEN GEAR BOX TO BE STRIPPED CAUSING THE BOLT TO BE LOOSE. THE CORRECTIVE ACTION WOULD BE FOR MAINT TO AVOID 'XTHREADING' OR STRIPPING BOLTS WHEN INSTALLED, OR CORRECTLY REPLACING OR REPAIRING THE STRIPPED BOLT OR SOCKET.
