Narrative:

Small aircraft X landed runway 28, turned off at the end then cleared air carrier X for takeoff runway 22. Ground control held air carrier Y on his frequency to taxi small aircraft Y past air carrier Y. I gave permission to cross runway 22 with small aircraft Y, turned to check air carrier X position to change him to departure and upon turning back to the approach end of runway 22 observed air carrier Y taxiing onto the runway (runway 22) west/O clearance. Supplemental information from acn 174644: at that point we were told to taxi into position and hold on runway 22. From our hold point short of taxiway delta to the time we set our brakes in position on runway 22, we were monitoring tower frequency.
