Narrative:

After takeoff runway 28R at pit, given clearance to 360 degree heading and 5000 ft. Advised of traffic above. Shortly thereafter issued further clearance to turn right to 090 degree heading, traffic's a cessna (310), proceed direct to tyrone when able maintain 8000 ft. Passing approximately 6000 ft ATC (pit departure control 124.75) requested present altitude, first officer responded 6000 ft for 5000 ft. ATC then stated we were only cleared to 5000 ft. ATC then cleared to us to maintain 14000 ft, continued to destination without further incident. No conflict noted on TCASII or stated by ATC. I believe the controller may have unintentionally misstated the altitude. However would readily acknowledge that having been awake since XA30 am, on duty for over 12 hours, having spent all day flying in moderate to severe turbulence, (phl-rsw-pit-pvd) having been unable to deplane passenger for over 25 mins because the jetway failed. Having a long delay due to a late inbound aircraft, having an aircraft then which had not been 'delayed' having a further maintenance delay to replace a slide which was not properly packed. Then after finally getting passenger aboard for last leg. Could not get a pushback crew because of a shift change when we finally started to taxi and had requested deicing were told 'west' pad is closed for night, proceed to 'a' pad. At 'a' pad were told 'a pad is full and not going to deice any more aircraft,' we're shutting down for the night upon completing present aircraft. Complained they had been on duty for 18 hours. 1 sat on taxiway pleading with company to help us get deiced. They finally reopened 'west' pad. We turned around taxied back got deiced and departed near XB00. After takeoff entered moderate turbulence in a snow shower. I was manually flying the aircraft at the time the clearance was issued. It was first day back since feb/xx/95 (vacation). I was stressed and fatigued!

ATC THEN CLRED TO US TO MAINTAIN 14000 FT, CONTINUED TO DEST WITHOUT FURTHER INCIDENT. NO CONFLICT NOTED ON TCASII OR STATED BY ATC. I BELIEVE THE CTLR MAY HAVE UNINTENTIONALLY MISSTATED THE ALT. HOWEVER WOULD READILY ACKNOWLEDGE THAT HAVING BEEN AWAKE SINCE XA30 AM, ON DUTY FOR OVER 12 HRS, HAVING SPENT ALL DAY FLYING IN MODERATE TO SEVERE TURB, (PHL-RSW-PIT-PVD) HAVING BEEN UNABLE TO DEPLANE PAX FOR OVER 25 MINS BECAUSE THE JETWAY FAILED. HAVING A LONG DELAY DUE TO A LATE INBOUND ACFT, HAVING AN ACFT THEN WHICH HAD NOT BEEN 'DELAYED' HAVING A FURTHER MAINT DELAY TO REPLACE A SLIDE WHICH WAS NOT PROPERLY PACKED. THEN AFTER FINALLY GETTING PAX ABOARD FOR LAST LEG. COULD NOT GET A PUSHBACK CREW BECAUSE OF A SHIFT CHANGE WHEN WE FINALLY STARTED TO TAXI AND HAD REQUESTED DEICING WERE TOLD 'W' PAD IS CLOSED FOR NIGHT, PROCEED TO 'A' PAD. AT 'A' PAD WERE TOLD 'A PAD IS FULL AND NOT GOING TO DEICE ANY MORE ACFT,' WE'RE SHUTTING DOWN FOR THE NIGHT UPON COMPLETING PRESENT ACFT. COMPLAINED THEY HAD BEEN ON DUTY FOR 18 HRS. 1 SAT ON TXWY PLEADING WITH COMPANY TO HELP US GET DEICED. THEY FINALLY REOPENED 'W' PAD. WE TURNED AROUND TAXIED BACK GOT DEICED AND DEPARTED NEAR XB00. AFTER TKOF ENTERED MODERATE TURB IN A SNOW SHOWER. I WAS MANUALLY FLYING THE ACFT AT THE TIME THE CLRNC WAS ISSUED. IT WAS FIRST DAY BACK SINCE FEB/XX/95 (VACATION). I WAS STRESSED AND FATIGUED!
