Narrative:

Pilot log write-up: yaw damper switch went to off and yaw damper light illuminate in-flight switch was placed to on operation normal. I performed bite check of yaw damper coupler. Bite check was performed per maintenance manual 22-12-32/501 for B737-300/400 bite showed 'pr fault.' maintenance control suggested replacing coupler. I replaced coupler, performed test per maintenance manual 22-12-32. Bite showed 'pr fault.' called maintenance control for MEL. Per MEL special procedure 22-3, the pr valve has to be switched from normal position to high pressure position. After gaining access to valve, found valve already in high position. This meaning previously accomplished, and not set back to normal. Notified maintenance control, aircraft released with new coupler changed for troubleshooting and on MEL 22-3. Logbook entry reads 'special procedure 22-3 accomplished previously.'

Title: A B737-300 DISPATCHED IN NON COMPLIANCE WITH A YAW DAMPER LOGBOOK RPT THAT WAS NOT CORRECTED AND DEFERRED PROPERLY.

Narrative: PLT LOG WRITE-UP: YAW DAMPER SWITCH WENT TO OFF AND YAW DAMPER LIGHT ILLUMINATE INFLT SWITCH WAS PLACED TO ON OP NORMAL. I PERFORMED BITE CHK OF YAW DAMPER COUPLER. BITE CHK WAS PERFORMED PER MAINT MANUAL 22-12-32/501 FOR B737-300/400 BITE SHOWED 'PR FAULT.' MAINT CTL SUGGESTED REPLACING COUPLER. I REPLACED COUPLER, PERFORMED TEST PER MAINT MANUAL 22-12-32. BITE SHOWED 'PR FAULT.' CALLED MAINT CTL FOR MEL. PER MEL SPECIAL PROC 22-3, THE PR VALVE HAS TO BE SWITCHED FROM NORMAL POS TO HIGH PRESSURE POS. AFTER GAINING ACCESS TO VALVE, FOUND VALVE ALREADY IN HIGH POS. THIS MEANING PREVIOUSLY ACCOMPLISHED, AND NOT SET BACK TO NORMAL. NOTIFIED MAINT CTL, ACFT RELEASED WITH NEW COUPLER CHANGED FOR TROUBLESHOOTING AND ON MEL 22-3. LOGBOOK ENTRY READS 'SPECIAL PROC 22-3 ACCOMPLISHED PREVIOUSLY.'
